We at Perspectives Counseling Centers want to be courteous of your busy schedule and know that it is difficult to come earlier at times to fill out the initial paperwork, so if it suites your schedule better to download the paperwork and complete it before your initial session please do so.

Please also keep in mind that we have separate forms for adult clients and minor clients.
